ADHD Diagnostic Evaluation Criteria

• A diagnostic interview with a qualified mental health professional (cannot be a CMHS clinician)
• Relevant medical, mental health, and medication history
• History of presenting symptoms related to ADHD
• Description of current functional limitations resulting from ADHD
• Information from collateral sources (such as school records)
• Neuropsychological testing report supporting the ADHD diagnosis
• A specific diagnosis of ADHD based on the DSM-V diagnostic criteria

• Documentation must be reviewed by a psychiatry clinician before an appointment can be scheduled. When the records have been received and reviewed, a determination will be made regarding the appropriateness of the evaluation and the eligibility of the student to receive treatment at CMHS. If appropriate, the student will be offered an initial evaluation appointment. If the records are insufficient, the student will be directed to resources for further evaluation.
• The treating clinician may recommend a different course of treatment; acceptance for treatment does not ensure a continuation of the existing medication regimen.
• The student is required to keep regular appointments.
• The student must comply with any prescribed medical assessments. Periodic evaluation of blood pressure and pulse will be necessary. An EKG or other medical evaluation may be indicated. The student may be required to have their weight monitored, as stimulants cannot safely be prescribed if a student’s weight is not maintained at a minimum of 90% of their Ideal Body Weight (IBW).
• Students receiving medication treatment at CMHS are required to be in concurrent counseling unless the prescribing clinician agrees that ongoing counseling is not necessary.
