Incomplete Pre-Entrance Requirements
Resolve Your Immunization Hold
If you’ve been directed here from your Bursar page, you have an immunization hold on your student account. This means you have not yet completed all required pre-entrance health and immunization steps.
Follow the instructions below to find out what’s missing and submit the required information.

How to Resolve Your Hold
Step 1: Log into the Patient Portal
Go to go.tufts.edu/portal and log in with your Tufts username and password.
Step 2: Review Your Requirements
Once you’re logged in:
- Message Inbox
Read any messages we’ve sent you — they may include specific details about what you’re missing. - Immunization Tab
Enter the dates of your vaccines. Then, upload your official vaccine record under the "Upload" tab. - Forms Tab
Complete these required forms:- Pre-Entrance Health History Form
- TB Risk Assessment Form
Step 3: Submit Your Information
Be sure to both enter your vaccine dates and upload your vaccine record in the portal. Once submitted, your information will be reviewed.
Please allow up to 10 business days for your hold to be removed after submission.
Notes for Certain Students
- Students age 30 and over who plan to use the Medford Health Service are encouraged but not required to submit their immunization records.
- Students in Occupational Therapy programs must submit immunization records regardless of age.
If You Still Have Questions
If you still have questions after reviewing the Patient Portal and the health requirements page:
- Send a message through the Patient Portal to Immunization Reviewer.
- Call Health Service at 617-627-3350.
What Happens If You Don’t Resolve Your Hold?
You won’t be able to register for classes until your immunization hold is cleared. Please complete the steps above as soon as possible.
