Incomplete Pre-Entrance Requirements

To review your missing health and immunization requirements, log into the Patient Portal at go.tufts.edu/portal:

Review your Message Inbox
Review the Immunization tab
For immunization compliance, first click on the immunization tab to enter the immunization dates; then, upload your vaccine record under the Upload tab.
For the pre-entrance health history form and the TB risk assessment, look for the corresponding form under the Forms tab.

Although vaccination to COVID-19 is not longer required, we strongly recommend that students stay up to date with vaccination in order to stay healthy, avoid missing classes, and safeguard the health of the Tufts community. Many local pharmacies offer COVID-19 vaccines or you can visit https://www.vaccines.gov to search locations.

The CDC recommends several ways you can look for COVID-19 vaccination providers near you in the United States:

Visit Vaccines.gov to find vaccination providers near you. In some states, information may be limited while more U. S. vaccination providers and pharmacies are being added.
Text your ZIP code to 438829, or phone 1-800-232-0233, to find vaccine locations near you in the United States.
Check your local pharmacy's website to see if vaccination appointments are available. Find out which pharmacies are participating in the Federal Retail Pharmacy Program.
Contact your state health department to find additional vaccination locations in the area.
Check your local news outlets. They may have information on how to get a vaccination appointment.
